New Patient Forms

  • - Prior to your first appointment with us, be sure to complete the following new patient forms. If not completed before your appointment, arrive 15 minutes early to complete them onsite.

Patient Portal

  • - View information about your medications, upcoming appointment, test results etc.
  • - You can request a refill
  • - Pay a bill
  • - Message your provider
Medication Refill

  • - You can request a refill through the patient portal, or by calling our office.
  • - The best time to call our office for refill requests is between 10AM-Noon.
  • - If we miss your call we will respond to your message before the end of the same-day, but may NOT be answered before your pharmacy closes.
  • - We recommend that you submit refill requests 48 hours ahead of time whenever possible.
After Hours Care

  • - For medical emergencies call 911 or head to the emergency room
  • - For non-emergent, but urgent care needs, visit Hometown Urgent Care to find the closest clinic near you.
  • - For any after hours needs that are non-emergent or urgent, please contact us during normal business hours to assist you.

Prepare For Your Visit

  • - Be sure to bring your Driver's license or ID and insurance card
  • - If you use reading glasses regularly bring them with you so you can review documents easily.
  • - Bring your medications with you.
  • - Bring a list of your vitamins and supplements.
  • - Be sure to have recent healthcare information including prior surgeries, recent lab testing, hospital stays, etc.
